چکیده

A 28-year-old man with no relevant medical history was admitted to our hospital for fever occurring early in the evening, weakness, dyspnea, and headache of 2 months’ duration. Blood culture was persistently positive for Streptococcus sanguinis (5/5). Upon admission, a diastolic murmur that had not been detected previously was found. Transthoracic echocardiography was performed and showed a large aneurysm in the noncoronary leaflet of a bicuspid aortic valve, severe aortic insufficiency, and left ventricular dilation (Figures A and B). Transesophageal echocardiography identified perforation of the aneurysm as the cause of aortic insufficiency (Figures C-H). After the patient had completed antibiotic treatment and blood cultures tested negative, a decision was made to operate due to persistent dyspnea in the presence of severe aortic insufficiency. The same findings were observed (Figure I) as described above. Aortic valve aneurysms are rare, with fewer than 15 cases reported in the literature. Although the most probable cause is infectious endocarditis, this disease has been demonstrated in only a few cases. Our case met 2 of the major Duke criteria and 2 minor criteria for the diagnosis of definite infectious endocarditis: positive blood culture, new valvular insufficiency, fever, and predisposing heart disease. Ultimately, the valve culture was sterile, but surgery was done after completing antibiotic treatment and achieving negative blood cultures. As in other published cases, there was an absence of characteristic images of endocarditis accompanying the aneurysm. Therefore, an isolated finding of aortic aneurysm advises assessing for infectious endocarditis to rule out the presence of this condition. Figure.

Perforated Mitral Valve Aneurysm in the Posterior Leaflet without Infective Endocarditis

Aneurysm of the mitral valve, although uncommon, occurs most commonly in association with infective endocarditis of the aortic valve and true mitral valve aneurysm is a rare cause of mitral regurgitation. We report a case with perforated mitral valve aneurysm in the posterior leaflet without concurrent infective endocarditis initially mistaken diagnosis of cystic mass, which was confirmed at op...

INFECTIVE ENDOCARDITIS IN CHILDREN

A total of 14 cases of infective endocarditis (IE) in children aged 6 months to 10 years were seen from December 1987 to December 1992 at the pediatric unit of Ayatollah Taleghani Medical Center. The majority of patients (12 of 14) were between 5 and 10 years of age. Acyanotic congenital heart disease was known to preexist in 78.6% and rheumatic valvular heart disease in 21.4% of cases. Or...

Infective Endocarditis of the Aortic Valve with Anterior Mitral Valve Leaflet Aneurysm.

Mitral valve leaflet aneurysm is a rare and potentially devastating complication of aortic valve endocarditis. We report the case of a 48-year-old man who had endocarditis of the native aortic valve and a concomitant aneurysm of the anterior mitral valve leaflet. Severe mitral regurgitation occurred after the aneurysm perforated. The patient showed no signs of heart failure and completed a 6-we...

Perforated mitral valve aneurysm associated with Libman-Sacks endocarditis.

The perforation of a mitral valve aneurysm is a rare disease which induces acute mitral regurgitation and is usually induced by infective endocarditis; however, in this case report, acute heart failure was caused by a perforated mitral valve aneurysm that was speculated to be due to Libman-Sacks endocarditis with systemic lupus erythematosis and secondary anti-phospholipid syndrome. Mitral valv...

Neisseria sicca Endocarditis Complicated by Intracranial and Popliteal Aneurysms in a Patient with a Bicuspid Aortic Valve

We report a case of infective endocarditis due to Neisseria sicca complicated by intracranial and popliteal aneurysms and hepatic and splenic infarcts in a patient with a bicuspid aortic valve. No predisposing factor other than poor dental condition was found. The patient fully recovered after antibiotic therapy, aortic and mitral valve replacement, endovascular occlusion of the middle-cerebral...
